Applying to register a Medical Professional Corporation
Please review the instructions below carefully. To avoid unnecessary delays in registering your corporation, ensure that EACH STEP is followed.
Each corporation is required to provide the following documents:
1. CPSS CORPORATION APPLICATION - FORM A
- Complete the form(s) required for registration with the College. Form A - Corporation Permit Application
- Have that form signed by a Member (only CPSS-registered physicians are "members") who holds voting shares in the corporation.
2. INCORPORATION DOCUMENTS
a. Attach a copy of the incorporating documents that your corporation has already registered with the Corporations Branch of the Province of Saskatchewan:
- Articles of Incorporation (with any Schedules)
- Articles of Amendment (if Articles of Amendment have been filed).
b. Submit a copy of the filed Annual Return, if the corporation has been in existence for more than one year.
3. OTHER DOCUMENTS: In certain limited circumstances, other documents may have to be filed with the College. That should only be required in certain limited circumstances if there is a complex corporate structure or a complex trust structure associated with the professional corporation.
4. CORPORATION REGISTRATION FEES:
- Initial Registration Fee (new corporations only) $350
- Annual Permit Fee (all corporations) $150
(The total fee payable in the first year of registration will therefore be $500 and the fee for each year thereafter will be $150 or such other fee as may be set by the Council.)

- Certificate of Incorporation, Articles of Incorporation with Schedules
- Registration fees: payment of $500 for new corporations (or $650 if prior to December 31) OR $150 for corporation registration renewal.
