Requests for data, historical files or other information



General Information


Individuals wishing to obtain data and other current information* about the College can:

  1. Consult the Annual Report (contains various sets of data on Council and its activities for the year in question).
  2. Call the CPSS at 1 (306) 244-7355, or write to CPSS, 101-2174 Airport Drive, Saskatoon, SK  S7L 6M6  
  3. Submit a request by email.  Be as specific as you can regarding the data set you are looking for.    
  4. Submit a Request for Historical/Physician Information form.

PLEASE NOTE: The College is bound to respect the Health Information Protection Act (HIPA) as well as restrictions imposed by the Council on releasing information that the College considers to be confidential.  The College may therefore redact part of the content of any file before providing any information to you.  The College also cannot provide any guarantee concerning accuracy or completeness of the information that is held within the file.  A charge may be associated to your request depending on the nature of the information required.   The College may also be unable to comply with some requests due to time constraints and staff availability.

 

Information on Registered Physicians (Past and Currently Registered)

Current information on physicians registered with the College is available through the Physician Search module located on the top left-hand site of the College's website.


 

For information on physicians of a historical nature dating before 1999, please write to communications@cps.sk.ca.  Depending on the nature of your request, you may be asked to fill out a Request for Historical/Physician Information form.  

Additional information can also be obtained through the Provincial Archives of Saskatchewan, where many of the College's historical files have been relocated.
