Physicians with Prescribing Restrictions


The physicians listed below are subject to prescribing restrictions relating to PRP or other medications, as indicated.

Dr. Yakub Abu-Ghazaleh  July 26, 2019

Dr. Abu-Ghazaleh has entered into an undertaking which includes the following:

  1. He will work with his chronic pain patients to assist them to transition their care to another physician, and will discontinue prescribing prescription review medications to chronic pain patients within 12 months;
  2. He will review his prescribing to all chronic pain patients to assess whether the prescribing is appropriate, and if he concludes that it is not appropriate, he will alter his prescribing to what is appropriate;
  3. If after reviewing his prescribing to chronic pain patients he decides to continue prescribing opioid doses in excess of 90 morphine equivalents per day, he will obtain a signed acknowledgement that the patient is aware of the risks of such prescribing and the patient is either given a prescription for Naloxone or instructions on how to obtain a Naloxone kit.

Undertaking
 

Dr. Murray Davies  December 1, 2016 Dr. Davies has signed an undertaking with the College in which he has agreed that he will not prescribe, order, dispense or administer any Prescription Review medication or Pregabalin (Lyrica), Tramadol, Zopiclone, Baclofen or Cyclobenzaprine

Dr. Davies is able to prescribe opiates in two circumstances:

1. To inpatients who require opiates for acute pain management only of his or her condition.
2. To emergency room patients when he is on call who require opiates for acute pain management only.


Dr. Russel Knaus  2010

Dr. Knaus is subject to a penalty order which prohibits him from prescribing thyroid replacement therapy or Lugol’s iodine.
 

Dr. Narinda Maree  June 21, 2019 Council has prohibited Dr. Maree from prescribing any medication that is listed in the Prescription Review Program of the College.
 
Dr. Oluwatomi Mitchell September 4, 2017

Dr. Mitchell has signed an undertaking with the College that she will not prescribe any opioids to any patient.
 

Dr. Andre Van Der Merwe  August 29, 2017

Dr. Andre Van der Merwe has signed an undertaking with the College that he will not prescribe any Prescription Review medications, including testosterone or its derivatives, to any patient.
 

Dr. Jordan Velestuk February 2, 2023

Dr. Velestuk may only prescribe, dispense, or order Prescription Review medications while practicing with the Saskatchewan Health Authority in Moosomin or with Northern Medical Services.
